The Practice welcomes new patients who live within our Practice area. There are some areas of both East and West Sussex that do not fall within our catchment area. Check your post code here
Our Receptionists will help you to register when you attend the practice. We do ask that you bring with you, if you have one your medical card and proof of identity and your place of residence, i.e. passport/driving licence and utility bill. We will also ask you to provide your NHS number, which is available from your current Practice. If you are unable to provide us with these documents this will not prevent registration, you may however, be asked to provide an alternative form of ID.
If you would like to register with us, you can download all the necessary forms here and if joining as a family we will need one for each individual. Please complete the forms in full and bring them to the surgery together with the identity documents detailed on the front page of the New Patient Registration Form. Should you have any difficulty providing this documentation don't hesitate to contact the practice reception team on 01273 834388 to talk through alternative options.
