Care Co-ordinators

If you are living with a medical condition and require some additional support - whatever it is - we are here to help.

The Practice is offering a new service for our patients, delivered by our Care Co-ordinators.  The service is designed to help support patients with long term conditions in a practical way to help them access the many varied services, available both within the NHS and the local community.

Services we commonly help with include:

  • Transport links - Blue Badge, taxi vouchers, etc
  • Carers Support
  • Equipment for the home
  • Befriending
  • Continence issues and many more

If you feel you would benefit from this service, or someone you care for would benefit, please make an appointment or ask for a telephone call with one of our Care Co-ordinators.

Dementia Support Clinics

These are held on a regular basis for those with or affected by dementia.  Clinics are held at Hurstpierpoint and Hassocks and run by Kim Hall who is a dementia support worker with the Alzheimer's Society. 

Appointments last 20 minutes and can be pre-booked by contacting one of the Care Co-ordinators on 01273 834388.

Social Prescribing

Social prescribing is when health professionals refer patients to support in the community, in order to improve their health and wellbeing. The concept has gained support in the NHS organisations of the United Kingdom as well as in Ireland and the Netherlands.  As a practice we would actively encourage our patients to look at engaging with external support projects or activities within the community that may benefit their wellbeing. 

The following link relates to the Stanmer Park Restoration Project which welcomes volunteers who would benefit from engagement with other like-minded people and enjoy spending time in a natural environment.

Park Run activities which are free weekly 5km timed runs, are open to everyone, free, and are safe and easy to take part in.  These events take place in pleasant parkland surroundings and we encourage people of every ability to take part; from those who enjoy walking to those that will be taking their first steps in running; all are welcome.  You may find you bump into members of the Practice Team, many of which are keen runners.
